Diagnosis codes are required when processing insurance and orders. The Diagnosis Code (ICD-9 code) is required with the procedure code on the CMS/HCFA 1500 Form; the most commonly used diagnosis codes are included in My Vision Express.
• For orders, up to four diagnosis codes can be added to a particular order. If you generate an insurance record from this order the diagnosis information is automatically populated in the Insurance Screen and the CMS/HCFA form from the order.
• For insurance, you can directly enter diagnosis codes into insurance record and they will be printed on the CMS/HCFA 1500 Form.
• To add new Diagnosis Codes:
1. Follow the menu item sequence … File | Setup | Diagnosis Codes.
2. In the Diagnosis Code Screen click the Add Button located on the upper right part of the screen.
3. In the Diagnosis Code text box, enter the number of the new diagnosis code.
4. In the Description text box, enter the description of the new diagnosis code.
5. If you would like to add another Diagnosis Code go to step 2, otherwise click Save then Close from the menu toolbar.
